If you are an incoming or current student, the first step in registering with Disability Services is to self-identify as a student with a disability and request accommodations.
The review, registration, and implementation process can take 2-4 weeks, sometimes longer, depending on the nature of the accommodation request, supporting documentation, and appointment availability during peak times in the semester. Approved accommodations are applied proactively rather than retroactively, so students are encouraged to begin this process early. The same timeline applies for students who submit additional documentation so that their file may be reviewed again for accommodation updates. All communications regarding your accommodation requests will be sent to your Mount email address.
Step 1- Complete this Application
Answer all questions as thoroughly and completely as possible using your MSMU Credentials. The information you provide is helpful to us in determining your needs and developing accommodations.
Step 2 - Submit Documentation
Submit medical documentation from a licensed professional to support your request. Your medical provider can complete our
Documentation Request Form
Please review our
Documentation Guidelines, and contact us at any time with questions.
If you are not able to upload documentation right away, you may provide it at a later date.
Please note: your file will be held as “pending” until we receive your documentation. However, you are welcome to schedule an appointment with a Disability Services team member if you have any questions or concerns about the process.
Step 3 - Meet with a Disability Services staff member
After we review your documentation, a Disability Services staff member will contact you to schedule a meeting. This first meeting will be an interactive session designed to give our office a narrative of your disability and to determine your access and accommodation needs. We will review all accommodation policies, the next steps, and responsibilities for your accommodation request. All accommodation requests are on a case-by-case basis and evaluated so that they are reasonable and do not fundamentally alter curriculum or state licensure requirements.
A Note on Confidentiality:
Medical documentation provided through myDSP is reviewed only by Disability Services staff. Medical documentation provided to Disability Services by mail, in-person delivery, fax, or by uploading to myDSP, is stored separately from student's academic records.
